PHASE 1B COVID-19 VACCINATION PLANS BEGIN IN OAK PARK ON FEBRUARY 1, 2021:
PRE-REGISTRATION ASSISTANCE AVAILABLE THROUGH
OAK PARK TOWNSHIP SENIOR SERVICES
The Village of Oak Park, Illinois Department of Public Health has launched an online vaccine pre-registration form for individuals who live or work in Oak Park and may be eligible to receive a COVID-19 vaccine as part of Phase 1b of the state's vaccine distribution plan.

Should you need help completing the online form or if you don't have email, please call Oak Park Township Senior Services at 708-434-4298 weekdays from 9 a.m. to 5 p.m. for assistance. All other vaccine allocation questions should be sent to the Health Department via email at email@example.com.
The Oak Park Department of Public Health has been designated a COVID-19 vaccine provider and is receiving doses directly from the state. Allocation of the vaccine is determined by protocols outlined by state officials at Illinois Department of Public Health and federal officials at the Centers for Disease Control and Prevention.
Effective Jan. 25, individuals in Phase 1a and Phase 1b of the state vaccine distribution plan are eligible for vaccination. This includes health care workers, residents of long-term care facilities and congregate homes, individuals age 65 and older and frontline essential workers such as educators, licensed daycare staff, grocery store clerks, pharmacy workers, transit employees, mail carriers and delivery drivers.
