We are proud to offer the GUIDE Comprehensive Dementia Care Model in partnership with Eli Health.
Eligible Medicare beneficiaries may receive personalized dementia care, caregiver support, and up to $2,563 annually in respite care services.

Oak Park Township Open During Renew the Avenue Construction
Oak Park Township services will continue during the Village of Oak Park’s Renew the Avenue construction project. This project will replace more than 100-year-old infrastructure with new water and sewer pipes in and adjacent to Oak Park Avenue’s Hemingway District, followed by streetscape improvements.
While construction is underway, Township services will remain available with temporary service locations and access adjustments to ensure residents can continue receiving support beginning March 13. A full list of Township services and their temporary locations is available in this document.
Visiting During Construction
Street closures, parking adjustments, and access changes may occur as construction progresses. Residents are encouraged to call ahead before scheduled appointments to confirm access, service locations, and parking availability.

Elected Officials and Board Meetings
Oak Park Township is governed by an independently elected Township Board of Trustees. The Township Board consists of the Township Supervisor and four Trustees, elected at large from the township. The Clerk and Assessor are elected simultaneously with the Board. The Assessor does not vote. The Township Clerk votes only in the case of a tie vote to fill a vacancy in a township office.
