CARE COORDINATOR/INTAKE SPECIALIST - FULL TIME
Oak Park Township is seeking a full time Care Coordinator/Intake Specialist to serve as a member of the Senior Services team to conduct comprehensive initial intakes, Options Counseling, Adult Protective Services (APS) investigations, and hospital based coverage.  The ideal candidate will work well independently, have knowledge of community- based services for older adults, and experience working with the Adult Protective Services program.  This is a Tuesday-Saturday, full time position with multiple components.

The Care Coordinator/Intake Specialist will be responsible for:

  • Conducting initial intake for the Adult Protective Services (APS), Community Based Services and Geriatric Case management programs
  • Providing information and referrals to callers
  • Developing Options Counseling care plans for those needing assistance in their deliberations to make informed long-term support choices; keeping in mind the participant and their stated preferences, strengths, and values
  • Investigating reports of suspected abuse, neglect, or financial exploitation (ANE) of older adults (60+) or those aged 18-59 with a disability
  • Completing hospital-based programming including screenings on Saturday, backup screenings as needed
  • Conducting outreach at the local hospital locations
  • Documenting service provision in a timely manner and in accordance with the regulations of the program.

Requirements of the position:

  • Bachelor’s degree required, BSW preferred
  • One year of experience in service provision to older adults
  • Experience with Microsoft Office, database, and applicant tracking systems
  • Current driver’s license and proof of auto insurance
  • Basic knowledge of Older Americans Act Services, Medicaid, Medicare, Social Security, SSDI, and the Community Care Program is preferred

CERTIFIED FOOD SERVICE STAFF LEADER – PART TIME
(32.5 hours/week 7 a.m. to 1:30 pm)

Oak Park Township is seeking a part time Food Service Staff Leader to serve as a member of the Senior Services team to help the Township achieve its mission of: 1) addressing the needs of our community by providing nutritious meals and opportunities for recreation, education, and socialization for older adults; 2) assisting with the operation of the nutrition site in an open, inclusive, and fiscally responsible manner while maintaining or improving service quality to staff, program participants, vendors and other partners; 3)  assisting with opportunities for increased collaboration with community partners and providing volunteer opportunities to enhance service provision while conserving funds.

The Food Service Staff Leader will:

  • Coordinate kitchen operations, oversee responsibilities of the kitchen staff and volunteers, ensure adherence to food safety guidelines throughout the service day
  • Monitor and provide feedback to kitchen staff and volunteers to aid them in successful daily operations of the nutrition site.  Troubleshoot to resolve issues efficiently and effectively as they arise.
  • Create a friendly, open, and welcoming environment by engaging with participants, volunteers, and staff.
  • Act as a team player by providing staff coverage for other members of the Nutrition team as needed.
  • Oversee food service equipment and supplies, maintain inventory and alert Nutrition Program Manager of needs and concerns

The position requires a High School Diploma, ServSafe Food Service Sanitation Certification and one year experience as a team leader in a food service environment.  Successful candidates must possess good interpersonal skills, demonstrate tact, and show sensitivity to the needs of older adults.  Experience with Microsoft Office, databases and applicant tracking systems a plus.

STRATEGIC PREVENTION FRAMEWORK ASSISTANT COORDINATOR

Oak Park Township seeks an individual to assist with the Strategic Prevention Framework – Partnerships for Success grant to reduce underage drinking in Oak Park and River Forest. This position will assist the Prevention Manager with scheduling and facilitating community coalition meetings, creating and submitting reports to the Illinois Department of Human Services, coalition members and leadership team.

Necessary skills include project management, organization, written and oral communications. The successful candidate will have the ability to meet deadlines, track activities, follow through on tasks, and communicate clearly and concisely. Experience with grant management is preferred. Oak Park Township is a unique unit of Oak Park local government, providing human services to vulnerable citizens of our community: at-risk youth needing guidance to improve their chances for a successful life, senior citizens needing support to maintain their independence and financially struggling residents who need monthly assistance and job seeking help.  In addition, the Township provides funding for mental health agencies and assistance with property tax inquiries and appeals.
